Almost never. An applicant tracking system is a database with a workflow attached — it stores applications, parses them into fields, and lets a recruiter search and filter. In most configurations it does not score your resume and bin it before a person looks. The widely repeated claim that 75% of resumes are rejected by robots is not a research finding; it is a marketing line popularised by companies selling software to beat the software. The real filter is a human spending five to twenty seconds deciding whether to keep reading. What an ATS genuinely can do is fail to read you properly, so a recruiter searching for a skill you have never surfaces your application. That is a parsing problem, not a rejection, and it is fixable in about ten minutes.
Structure, not styling. The reliable breakers are a multi-column layout, because older parsers read cell by cell rather than left to right and your dates end up attached to the wrong employer; layout tables, for the same reason; text boxes and images with words baked into them, which are invisible to a parser, so contact details placed there simply do not exist; and creative section headings, because parsers use words like Experience and Education to decide which text belongs in which field. A scanned resume with no text layer is the worst case — the file looks fine to you and is empty to everything else. What matters far less than people claim: your font, whether you submitted a PDF, and whether you matched a keyword letter for letter.
You need a plain one, which is not the same as an ugly one. A single column, standard headings, real text rather than graphics, bullets as actual list items, and contact details in the body of the first page rather than in the header or footer. That is the whole specification. Be suspicious of any template sold as ATS-optimised that puts your skills in a sidebar — that sidebar is a second column, and it is the most common reason a resume parses into nonsense. The same plain structure serves the human skim too, because a recruiter reading top to bottom gets your current title, then your most recent work, in the order you intended.
